Steps to Getting Services

  1. Create your unique ID/Password to log-on to the site – From the homepage select Member Login”.  If you have not already, please set up your unique ID/Password.
  2. Enter New Patient Information- from the Main Menu choose “New Member Information” Please complete all the boxes so we can know something about you.  It will help with your treatment!
  3. Enter Your Vitals – Choose “Enter Patient Vitals” from the Main Menu. If we need to Prescribe Medication, we will need to know this very important information about you.  You can take your Blood Pressure, weight yourself and measure your height and list any comments you like to give us about your vitals in the comment box.
  4. Read and Sign your Intake and Consent Forms-Choose the “Request Copies of Intake Forms” to have copies of each sent to you. Before we proceed, we will need you to read and sign our Intake forms, and patient consent forms.  Simply enter the email address where you want them sent, and the person to send them to.  Once you sign in the appropriate boxes our Agency will be able to retrieve copies via DocuSign.
  5. Agency contacts Client within 24 hours – Using the Email address given to us in the New Patient Information page, one of our Medical Assistants will contact you to approve your date and help you choose a Clinician. 
  6. Agency Sends Invoice – Before we start services, we will need for you to pay your invoice.  An invoice will be sent to you within 1 hour after speaking with you to the email contained within the “New Patient Information” page.  You must review the invoice carefully to make sure the information contained on the invoice is correct.  The invoice will list:
    1. Time and Date of the Session
    2. Name of Clinician you have chosen
    3. Amount due
  7. Approve Invoice – Simply email us back referencing the Invoice number letting us know you approve.
  8. Pay Invoice – Log on to the site and choose the “Make Payment Option” listed under the Enter New Patient Information” tab from the main menu.  Choose the appropriate amount and complete the information requested in pay pal.
  9. Agency Sends Confirmation – Within 24 hours you will receive an email with the following information:
    1. your confirmation number as proof of payment
    2. your date and time of Tele-Med session
    3. your Clinician Name
    4. your Doxy.Me link and code for your Tele-Med Session
    5. Our Nurse contact information to help with questions or concerns with your medication and to help with possible problems you may encounter with your prescription.  The nurse will be available for 24 hours after the session.  After 24 hours the client will be responsible for the cost of the Nurse.
  10. Call from one of Telxas Support Techs 48 hours prior to the session (first session only) One of our support techs will call you and conduct a live test with you.
  11. On Day of Appointment Click on link Using the link provided and code, log-0n on the date and time agreed upon to meet your Clinician.
  12. Need Copy of your Session – Email us and request copy.  Copies are sent out within 48 hours.
